Instructions for completing the form:
1. Right click on image (registration form)
2. Left click Save As a document or on your desktop
3. Go to the document fill it out
4. Under the File tab click print
For Google Chrome and Microsoft Edge users
Directions for printing:
1. Left click on form
2. Right click on SAVE AS
3. Save As Registration Form and save to Desktop
4. Click out of Website
5. Click on form saved to desktop
6. Go to File in navigation and click on print
7. Fill in form and mail it with your check.
